  • Report Processing

    Guidelines for use of Unusual Occurrence Forms, Disciplinary Reports, and memorandums. Requires reporting after use of a weapon, restraints, or other use of force. Event Forms are to be used for criminal incidents and inmate injuries. Reports are to be signed by a supervisor on the day of the incident.

  • Unusual Occurrence – Reporting and Notification

    Procedure for reporting “unusual occurrences” and a list of personnel to be notified. Requires incidents of serious inmate injury be analyzed for corrective action. Defines “significant occurrences” as death, serious injury or illness of staff or inmate; escapes or other situations that could draw public attention to ACSO.

  • Computer Hardware/Software Problems

    Procedure for documenting and correcting problems related to computer hardware and software.

  • Radio Accountability

    Policy for staff use of radio system. Hand held radios are equipped with a locator system and may not be stored in personal lockers. Lists the channel designations for various facility locations.

  • County Issued Personal Communication Devices

    Policy for issuance and accountability of county-issued Personal Communication Devices (PCD), which are to be carried by select staff at all times while on duty. Staff are additionally “strongly encouraged,” but not required, to carry PCDs while off-duty.

  • Repair and Minor Construction Report

    Policy requiring efficient repairs and minor work order construction. Requires expedited and timely facility repair requests, Observation Reports, Inter Departmental Service Orders, and Special Project Requests. Details the procedures for requests including normal maintenance, emergency repairs, special project requests, and telephone repair.

  • Facility Plant Maintenance

    Requires Building Maintenance Division of the General Services Agency (BMD/GSA) to provide a preventive maintenance program for building systems and equipment to minimize out of service time due to failure and reduce costly breakdown repair. Outlines procedures for maintenance requests, routine maintenance, and inspections.

  • Emergency Cell Water Shut-Off

    Procedures at Dyer and SRJ for stopping the flow of water in an emergency until repairs are made.

  • Emergency Services Delivery System

    Procedure for alternative emergency delivery system of services to inmates at SRJ if the Automated Guided Vehicle System (AGVS) becomes inoperable or incapacitated due to failure of the central master control system, or other system failure.
